NMDP Advanced Cell Therapy Lab Solutions

NMDP ACTL Solutions offers fresh or cryopreserved product processing of HPC, Marrow; HPC, Apheresis; and MNC, Apheresis products through NMDP partner labs. We are here to complement or augment your cell therapy lab capabilities as the need arises for your transplant center. Examples of how transplant centers have used our services include: 

• During times of staffing shortages or training needs to alleviate large volumes of products coming into their labs
• To accommodate weekend processing needs when donor availability is prohibitive to alternative collection timing  
• For capabilities not available at the transplant center’s lab, such as red cell reduction of a marrow product or CD34+ selection

Your transplant center still needs to obtain approval from NMDP to proceed with cryopreservation. The donor center or registry and donor also needs to provide approval. However, you do not need additional approval to use any of the other services through ACTL Solutions. 
Let your case manager know as early as possible if you plan to use one of our ACTL Solutions products. This allows enough time for the donor center or registry to counsel the donor and gain consent from them if needed and/or for completion of other necessary items.

No, there is no difference in the storage/shipping pricing based on the donor's collection location or the location of the transplant center, unless otherwise notified. The courier and collection cost will, however, be different depending on supplier.

Unless noted on the fee schedule and/or notified otherwise, donor workup, product procurement (collection and courier), cryopreservation processing (if applicable), storage (if applicable) and courier/shipment charges are delayed until after the product has shipped from our partner facility to your center.

The fee schedule accounts for instances when charges will be posted at the time of collection or pre-shipment, regardless of product disposition. Your case manager will notify you about this possibility for your center’s consideration at the time of your request.

We are committed to eliminating barriers to a patient’s transplant. Removing financial risk for our transplant center partners is one way we can help. In the unlikely event your patient is unable to proceed to transplant, unless noted otherwise, your center will not be charged for the product procurement or cryopreservation processing and storage as long as the cryopreserved product has not shipped to your center. 

As part of the consent process for donors, the donor center or registry will ask the donor for consent to keep their cells stored and available for use by a different patient. If your patient cannot use the product and the donor has consented, the product will be listed in MatchSource on the Adult Donor Cryopreserved Product (ADCP) list so it can be used by another patient. 

Yes. Cryopreserved products requested with custom CD34+ dosing that aren’t used by the original recipient can still be listed for other searching patients if the cryopreserved product has not shipped to the transplant center and the donor consents.

Yes. We can complete product processing and/or cryopreservation for any donor, domestic or international, assuming the donor and donor center or registry approve. We will work with your center to consider the transport time and processing considerations of products arriving from international origins.

Your center can use our ACTL Solutions products when NMDP’s Related Donor Services program supports your related donor workup and collection. 

However, charges for the donor workup, collection, courier, cryopreservation (if applicable), product processing (if applicable), storage and shipment will be passed along to your center regardless of product disposition because we are unable to list related donor products in MatchSource on the Adult Donor Cryopreserved Product (ADCP) list for use by other searching patients.

Yes. The donor center or registry will have a conversation with the donor and get their consent for cryopreservation during the workup process. In addition, the donor will be notified if their cells are not used for the first intended patient.

There are several advantages for your center. In cases where you have identified a donor but have concerns about scheduling their donation to align with your patient’s transplant timeframe, consider proceeding with cryopreservation through ACTL Solutions. 

When leveraging ACTL Solutions for your cryopreservation needs, in most cases we will defer donor workup, collection, courier, cryopreservation, storage, and shipment charges until after the frozen product is shipped to your center. If your patient is unable to proceed to transplant and the cells have not been shipped, your center is not responsible for those charges unless stated differently on the fee schedule and/or notified by your case manager. This reduces the financial risk to your center. 

Additionally, if a cryopreserved product is released from the originally intended patient’s search, remains in storage and hasn’t shipped to your center, and the donor provides consent, the product can be listed in MatchSource on the Adult Donor Cryopreserved Product (ADCP) list and potentially used for another searching patient.

Lastly, our cryopreservation offering may be a good option if your center lacks the capacity to accept donor product for cryopreservation—such as during staffing transitions and extended training periods—or does not have experience with cryopreserving bone marrow. We also assist in critical emergency situations as needed—such as during extreme weather conditions or unexpected issues that impact receipt at your center’s own laboratory.

Yes. Donor samples can still be collected on the day of donation and shipped directly to your lab. In addition, if a product is cryopreserved, it will ship with a minimum of 3x 0.5ml cryovials of the product. 

ISBT 128 labeling standards are used for all products processed at NMDP partner labs. These labels must meet FACT and AABB requirements.

Because an NMDP cryopreserved product could potentially be used for a recipient who is not the intended recipient at the time of collection, products cryopreserved by NMDP partner labs require labeling elements that will differ from those listed in MAN-00014, Cellular Product Labeling, Release, and Transport (Chapter 12 of the Transplant Center Manual of Operations) (Legacy A00185) (Opens in a new tab). 

NOTE: Due to the cryopreservation and storage process, it is appropriate to follow partial labeling practices as outlined by FACT and AABB for NMDP cryopreserved products.
